Corticosteroids




Corticosteroids are a class of steroid hormones that are produced in the adrenal cortex. Corticosteroids are involved in a wide range of physiologic systems such as stress response, immune response and regulation of inflammation, carbohydrate metabolism, protein catabolism, blood electrolyte levels, and behavior.



Glucocorticoids such as cortisol control carbohydrate, fat and protein metabolism and are anti-inflammatory by preventing phospholipid release, decreasing eosinophil action and a number of other mechanisms. Mineralocorticoids such as aldosterone control electrolyte and water levels, mainly by promoting sodium retention in the kidney. Some common natural hormones are corticosterone (C21H30O4), cortisone (C21H28O5, 17-hydroxy-11-dehydrocorticosterone) and aldosterone.

Uses


Synthetic drugs with corticosteroid-like effect are used in a variety of conditions, ranging from brain tumors to skin diseases. Dexamethasone and its derivatives are almost pure glucocorticoids, while prednisone and its derivatives have some mineralocorticoid action in addition to the glucocorticoid effect. Fludrocortisone (Florinef) is a synthetic mineralocorticoid. Hydrocortisone (cortisol) is available for replacement therapy, e.g. in adrenal insufficiency and congenital adrenal hyperplasia.
Synthetic glucocorticoids are used in the treatment of joint pain or inflammation (arthritis), temporal arteritis, dermatitis, allergic reactions, asthma, hepatitis, systemic lupus erythematosus, inflammatory bowel disease (ulcerative colitis and Crohn's disease), sarcoidosis and for glucocorticoid replacement in Addison's disease or other forms of adrenal insufficiency. Topical formulations are also available for the skin, eyes, lungs, nose, and bowels. Corticosteroids are also used supportively to prevent nausea, often in combination with 5-HT3 antagonists (e.g. ondansetron).
Typical undesired effects of glucocorticoids present quite uniformly as drug-induced Cushing's syndrome. Typical mineralocorticoid side effects are hypertension (abnormally high blood pressure), hypokalemia (low potassium levels in the blood), hypernatremia (high sodium levels in the blood) without causing peripheral edema, metabolic alkalosis and connective tissue weakness.[7]
Clinical and experimental evidence indicates that corticosteroids can cause permanent eye damage by inducing central serous retinopathy (CSR, also known as central serous chorioretinopathy, CSC). A variety of steroid medications, from anti-allergy nasal sprays (Nasonex, Flonase) to topical skin creams, to eye drops (Tobradex), to Prednisone have been implicated in the development of CSR.

Chcicken Pox


An acute,highly contagious disease that is caused by a virus & marked by eruptions on the sin. It mainly affects children ten and is one of the mildest of infectious fevers.


The eruptions apper as dark red pimples , first on the back and chest , thenspreading to the face , scalp and arms . The disease last about two weeks after the appearence of the spots . It produces a mild fever , severe irritation leading to a desire to scratch the spots and loss of appetite .If a doctor confirms a case o chicken pox , the child should be put to bed if he feels unwell and kept isolated until all the blisters have formed scab.

Try to stop the child from scratching the spots , for this could lead to secondary infection and scarring . To help reduce this risk , cut his fingernails short and make sure that his hands are thoroughly clean .

Few people can catch chicken pox more than once , but the same virus that cause chicken pox in children can cause shingles in adult . As a result a adult with shingles can give chicken pox to a child.

METHOD FOR INTRADERMAL INJECTION

Materials:

26g 3/8" needle
1ml syringe

The site for injection of Mantoux test is the inner aspect of the lower arm. Measure from the elbow about a finger length down, see diagram. The site should be free of skin irritation.

Cleanse area, hold skin taut, inject fluid (bevel side up). A small bleb, just beneath the skin will be evident.

Meclizine is an antihistamine and anticholinergic. It works by blocking a chemical messenger in the brain, which helps to reduce or prevent vomiting.

Meclizine is used to treat or prevent nausea, vomiting, and dizziness caused by motion sickness(as Draminex). It is also used for vertigo (or dizziness) caused by certain inner ear problems.

Cautions

Before taking meclizine

You should not take this medication if you are allergic to meclizine.

Before taking meclizine, tell your doctor if you are allergic to any drugs, or if you have:

  • asthma or other breathing disorder;

  • glaucoma;

  • an enlarged prostate; or

  • problems with urination.

If you have any of these conditions, you may not be able to use this medication, or you may need a dose adjustment or special tests during treatment.

FDA pregnancy category B. Meclizine is not expected to be harmful to an unborn baby. Tell your doctor if you are pregnant or plan to become pregnant during treatment. It is not known whether meclizine passes into breast milk or if it could harm a nursing baby. Do not use this medication without telling your doctor if you are breast-feeding a baby. Do not give this medication to a child younger than 12 years old.

Directions for safe use :

Use this medication exactly as directed on the label, or as prescribed by your doctor. Do not use it in larger amounts or for longer than recommended.

Take meclizine with a full glass of water.

The chewable tablet may be swallowed whole, crushed, or chewed.

To prevent motion sickness, take meclizine about 1 hour before you travel or engage in activity that causes motion sickness. You may take meclizine once every 24 hours while you are traveling, to further prevent motion sickness.

To treat vertigo, you may need to take meclizine several times daily. Follow your doctor's instructions.

Meclizine can cause you to have unusual results with allergy skin tests. Tell any doctor who treats you that you are taking an antihistamine.

Store meclizine(also other drugs) at room temperature away from moisture, heat, and light.

Side Effects

Get emergency medical help if you have any of these signs of an allergic reaction: hives; difficulty breathing; swelling of your face, lips, tongue, or throat.

Less serious side effects may include:

  • blurred vision;

  • dry mouth;

  • constipation; or

  • dizziness, drowsiness.
