A test developed by Bruce Ames to determine is a chemcial is a mutagen

Why the Ames test is so useful

a. it is fast: requires just a few days to complete

b. can be performed easily

c. many compounds can be tested at one time

d. it is very sensitive: can detect one mutation per billion cells

e. can detect mutagaens formed as a result of normal cellular function

Elements of Ames test

a. tryptophan requiring Salmonella typhimurium: requires tryptophan to grow

tryp- bacteria

b. smooth endoplasmic reticulum from mouse liver: contains liver enzymes that detoxify ingested material.

These enzymes can produce mutagens from normally nonmutagenic chemicals that are ingested.

Viral infections during pregnancy

• Healthy pregnant women are not at special risk for disease from CMV infection. When infected with CMV, most women have no symptoms and very few have a disease resembling mononucleosis.

• CMV remains the most important cause of congenital viral infection. For infants who are infected by their mothers before birth, two potential problems exist:

At birth most (85-90%) children are normal. However 5% to 10% of infants who are infected but without symptoms at birth will subsequently have varying degrees of hearing and mental or coordination problems
Of the remaining 10-15% generalized infection may occur, and symptoms may range from moderate enlargement of the liver and spleen (with jaundice) to fatal illness (see below)



Most women who are infected with parvovirus B19 infection during pregnancy have a satisfactory outcome.

• However, gestational parvovirus B19 infection has been associated with adverse consequences such as fetal death and occasionally hydrops fetalis resulting from viral replication in the bone marrow.

• It is estimated that parvovirus B19 infection in pregnancy causes fetal loss in 9 per cent of pregnancies in which infection occurs during the first 20 weeks and

• hydrops fetalis in 3 per cent of pregnancies in which infection occurred between 9 and 20 weeks.

• the risk of fetal loss in women with asymptomatic infection appears to be similar to that in women with a rash.

• Fetal infection without fetal loss or hydrops is common.

• There is no evidence of B19-associated congenital abnormality in the newborn or developmental abnormalities appearing later in childhood.

• In utero blood transfusions have been shown to be successful in treating B19 hydrops fetalis, Spontaneous recovery of hydropic fetuses may occur with subsequent delivery of a normal infant.



HSV infections during pregnancy are rarely associated with disseminated disease of the visceral organs of the mother, especially the liver.

• In the UK the incidence of neonatal herpes is ~1.65 per 100,000 live births.

• Primary or initial infection during the first or second trimester is not associated with a significant risk to the foetus. However congenital infection has been documented, the risk increases in the third trimester.

• Congenital infection can lead to foetal loss or overt disease in newborn infant.

• Most commonly neonatal infection is via the birth canal and disease is evident 3-21 days post delivery.

• If a mother has recently acquired genital herpes, the risk of transmission to the infant is 10 times greater than if she has a latent form of the virus.

Neonatal disease can manifest as three different forms of illness.

• a mild, localized infection of the skin

• CNS infection (high mortality)

• or a disseminated one (high mortality)



The risk to a foetus is highest in the first few weeks of pregnancy and then declines in terms of both frequency and severity, although there is still some risk in second trimester. The virus infects the placenta and then spreads to the foetus. If non-immune mothers are infected in the first trimester, up to 80% of neonates may have sequelae.

The sequelae of congenital rubella syndrome are:

• Hearing loss. This is the most common sequella of congenital rubella infection especially when the latter occurs after four months of pregnancy.

• Congenital heart defects such as ductus arteriosis and others

• Neurologic problems (psychomotor retardation, mental retardation, microvephaly)

• Ophthalmic problems (cataract, glaucoma, retinopathy, microphthalmia - table 1)

• intrauterine growth retardation

• Thrombocytopenia purpura (table 1)

• Hepatomegaly

• Splenomegaly

There may also be variety of other problems including bone lesions, pneumonitis etc.

In most cases, there is neural involvement - lethargy, irritability, motor tone problems, mental retardation, motor disabilities, abnormal posture, neurosensory hearing loss.





Varicella infection in pregnant women can cause severe chickenpox with increased risks of varicella pneumonia.

The fetus is at risk of Congenital Varicella Syndrome (CVS) if infection occurs in the first 20 weeks of pregnancy (risk 1-2%). CVS can cause a range of problems including limb shortening, skin scarring, cataracts and growth retardation.

Infection with varicella in the later stages of pregnancy can cause premature delivery or neonatal chickenpox infection. This is particularly serious if the mother becomes infected 7 days before birth. For these reasons, pregnant women exposed to VZV are offered IgG testing and the neonates are given immunoglobulin to help fight the infection.

ايه الهيئات المسؤولة عن الايليتس ؟؟
IELTS is jointly managed by the University of Cambridge ESOL
Examinations (Cambridge ESOL), British Council and IDP: IELTS
Australia.

أنواع الايلتس :
Academic and General Training
IELTS is available in two formats – Academic and General
Training.
The Academic Reading and Writing tests assess whether
a candidate is ready to study or train in the medium of English
at an undergraduate or postgraduate level. Admission to
undergraduate and postgraduate courses is based on
the results of these tests.
The General Training Reading and Writing tests are not designed
to test the full range of formal language skills required for
academic purposes, but emphasise basic survival skills in a
broad social and educational context. General Training is
suitable for candidates who are going to English-speaking
countries to complete their secondary education, to undertake
work experience or training programmes not at degree level, or
for immigration purposes to Australia, Canada and New Zealand.
It is the responsibility of the candidate to inform the test
centre whether they wish to take the Academic or General
Training Modules. Centres are not responsible for providing
this information. The General Training Module is not offered
at all test administrations.

Test Scores
IELTS provides a profile of a candidate’s ability to use English.
Candidates receive scores on a Band Scale from 1 to 9.

Tests

Each candidate takes four tests, one in each of the four skills –
Listening, Reading, Writing and Speaking.

1- Listening

The Listening test takes around 30 minutes. There are
40 questions and four sections.
The Listening test is recorded on a CD and is played
ONCE only.
During the test, time is given for candidates to read the
questions and write down and then check their answers.
Answers are written on the question paper as candidates listen.
When the recording ends, ten minutes are allowed for
candidates to transfer their answers to an answer sheet.

A range of native-speaker English accents are used in the
recordings which reflects the international usage of IELTS.
A variety of questions are used, chosen from the following types:
• multiple choice
• short-answer questions
• sentence completion
• note/summary/flow-chart/table completion
• labelling a diagram
• classification
• matching

Marking and assessment
One mark is awarded for each correct answer in the 40-item test.
A confidential Band Score conversion table is produced for each
version of the Listening test, which translates scores out of 40
into the IELTS 9-band scale.

2- Reading

The Reading test takes 60 minutes. There are 40 questions,
based on three reading passages with a total of 2,000 to
2,750 words

All answers must be entered on an answer sheet during the
60-minute test. No extra time is allowed for transferring answers.

Task types
A variety of questions are used, chosen from the following types:
• multiple choice
• short-answer questions
• sentence completion
• note/summary/flow-chart/table completion
• labelling a diagram
• matching headings for identified paragraphs/ sections of the
text
• identification of writer’s views/claims – yes, no or not given
• identification of information in the text – true, false or not
given
• classification
• matching lists/phrases

3- Writing

The Writing test takes 60 minutes. There are two tasks to
complete. It is suggested that about 20 minutes is spent on
Task 1 which requires candidates to write at least 150 words.
Task 2 requires at least 250 words and should take about 40
minutes.

Answers must be given on the answer sheet and must be written
in full. Notes or bullet points in whole or in part are not
acceptable as answers.

Marking and assessment
Each task is assessed independently. The assessment of Task 2
carries more weight in marking than Task 1.

• Coherence and Cohesion
• Lexical Resource
• Grammatical Range and Accuracy

Task 2 responses are assessed on:
• Task Response
• Coherence and Cohesion

4- Speaking

The Speaking test takes between 11 and 14 minutes and
consists of an oral interview between the candidate and an
examiner.
All Speaking tests are recorded.

Task types
There are three parts to the test and each part fulfils a specific
function in terms of interaction pattern, task input and candidate
output.
In Part 1 candidates answer general questions about
themselves, their homes/families, their jobs/studies, their
interests, and a range of familiar topic areas. This part lasts
between four and five minutes.

In Part 2 the candidate is given a verbal prompt on a card and is
asked to talk on a particular topic. The candidate has one minute
to prepare before speaking at length, for between one and two
minutes. The examiner then asks one or two rounding-off
questions.

In Part 3 the examiner and candidate engage in a discussion of
more abstract issues and concepts which are thematically linked
to the topic prompt in Part 2. The discussion lasts between four
and five minutes.

طرق استخدام الويب فى عمل البحث العلمى

الانترنت

ماذا تعني؟

الإنترنت ، أَو ببساطة الشبكة (النت) ، هي شبكة متاحة عالميا تتكون من شبكات حواسيب متصّلة مع بعضها “الشبكة هي مجموعة من أجهزة الحاسوب متصلة ببعض”.
قهي ببساطة شبكة الشبكات، تتكون من الملايين من الشبكات الأصغر ؛منزلية، أكاديمية، تجارية، وحكومية تحمل العديد من المعلومات والخدمات، مثل البريد الإلكتروني، الدردشة، نقل الملفات وصفحات الويب.

تاريخ الانترنت

نشأت هذه الشبكة في البداية واعتمدت فكرة اللا مركزية في ربط شبكات الكمبيوتر بعضها ببعض،قام بهذا العمل “مؤسسة المشاريع المتطورة” التابعة لوزارة الدفاع الأمريكية لتخدم كوسيلة إتصالات في حال حدوث كارثة نووية. إعتمدت وزارة الدفاع الأمريكية عمل هذه الشبكة والتي تعمل ببروتوكول NCP في العام 1969.
لم يكن لدى المهندسين الذين خططوا للشبكة في بداية عهدها أدنى تصور لما آلت إليه الشبكة اليوم،ويعزى نجاحها العملاق اليوم للا مركزية الشبكة أو بمعنى آخر لا يوجد جهة واحدة تسيطر على مجريات الأمور بشأن الشبكة.

أحداث مهمة:

1969 انشاء شبكة بين 4 جامعات أمريكية, الشبكة التي تمثل بداية الانترنت.
1972 أول ظهور للبريد الالكتروني.
1974 أول استخدام رسمي للاسم “انترنت”.
1976 استعمال الأقمار الصناعية في الانترنت واتصال أوروبا بالشبكة.
1992 ظهور خدمة الويب.
1994 بيتزاهت تقبل الطلبات عن طريق الويب و افتتاح أول بنك “افتراضي” على الشبكة.

وظيفة الانترنت

الوظيفة الوحيدة لشبكة الانترنت هي نقل البيانات فيمكن النظر الي شبكة الانترنت على أنها “وسيلة المواصلات” التي تستخدمها البيانات لتنتقل من مكان الى آخر.

كيفية عمل الانترنت

كما سبق وأوضحنا فالانترنت هو شبكة معقدة جدا من الأجهزة و الحواسيب، ولكن السؤال الذي يطرح نفسه،اذا قمت بارسال رسالة الى صديق في اليابان مثلا فكيف تصل اليه الرسالة دون أن تضيع في هذه الوصلات المعقدة؟

عندما تقوم بارسال بيانات عن طريق الانترنت فهذه البيانات تمر بعدة مراحل قبل أن تصل الى الهدف النهائي.

أولا يقوم حاسوبك بتقسيم هذه البيانات الى رزم صغيرة أو packetsكل رزمة مزودة بمعلومات عن العنوان النهائي المراد الوصول اليه.
بعد عملية التقسيم يقوم جهاز الكمبيوتر لديك بارسال هذه البيانات الى جهاز آخر يسمى الـ router هذا الجهاز لديه جدول يحتوي على خريطة للطرق “اذا صح التعبير” بمراجعة هذه الخريطة يعرف في أي اتجاه يرسل الرزمة حتى تخطو خطوة أقرب الى هدفها النهائي، و يقوم هذا الجهاز بدوره بتوجيه الرزم التي أرسلها جهازك الى router آخر أقرب الى الهدف النهائي,و الذي يقوم بدوره بتوجيه الرزم الى router آخر أكثر قربا الى الهدف النهائي وهكذا حتي تصل الرزم الى هدفها “حاسوب صديقك في اليابان” الذي يُعِيد تجميع الرزم ليحصل على البيانات في صورتها النهائية.

هذا المبدأ هو أساس عمل شبكة الانترنت بجميع خدماتها “البريد الالكتروني و الوب و غيرها”.

الويب

ماذا تعني؟

الوِيب (الشبكة العنكبوتية) هي مجموعة من الصفحات و المستندات ومصادر أخرى مرتبطة مع بعضها باستخدام روابط ، وهي من أكثر خدمات الانترنت استخداما, جميع موافع الانترنت ليست إلا جزء من الويب.

الفرق بين الويب والانترنت

تعمل الويِب فوق شبكة الإنترنت فهي جزء منها، لكن إنترنت تشمل الكثير غير الويِب,فكما سبق و أوضحنا أن الوظيفة الوحيدة لشبكة الانترنت هي نقل البيانات والويب ليس إلا شكل من أشكال هذه البيانات.

يمكن تشبيه الانترنت بشاحنة نقل على أحد الطرق السريعة ,مهمتها الوحيدة هي نقل البضائع “البيانات” بغض النظر عن طبيعتها من مكان الى آخر , أما الويب فيمكن تشبيهه بأحد أنواع البضائع المنقولة ,فجميع أنواع البضائع “البيانات” تحتاج الى وسيلة نقل وهي “الانترنت” ولكن اشتراكها في وسيلة نقل واحدة لا يعني بالضرورة تشابهها.

في الاستخدام الدارج يستعمل الويب والانترنت للدلالة على معني واحد وهو “الويب” وذلك لأهمية وضخامة المعلومات والبيانات التي توفرها هذه الخدمة خصوصا للمستخدمين المبتدئين، وذلك يشبه تماما اطلاق العامة على مدينة القاهرة اسم “مصر” رغم أن القاهرة هي جزء من مصر.

فوائد الويب

تعتبرالويِب أحد أكبر مصادر المعرفة في العصر الحديث، ملايين الصفحات والوثائق والكتب والمواقع بأكثر مما يتخيّل أي إنسان. نظريا يمكنك الوصول إلى أي معلومة عن كل شيء تقريبا. أهم مايميّز محتوى الوِيب هو حرّيته، فلم تفلح أي حكومة أو جهة في السيطرة على هذا المحتوى، وهو متاح للجميع في أي مكان.

ماذا تعني؟

محرّك البحث هو خدمة توفِّرها شركة أو موقع ما تتيح لك البحث في محتوىالويِب, بسبب الضخامة الشديدة لمحتوى الوِيب من المستحيل عمليا البحث داخل هذا المحتوى باستخدام الطرق التقليدية. في المقابل تقوم محرِّكات البحث بأرشفة هذا المحتوى (ليس كلّه في الحقيقة ولكن جزء كبير منه) وتوفير أدوات للبحث داخل هذا الأرشيف.

كيفية عملها

عملية الأرشفة

يقوم محرك البحث بهذه العملية بشكل يومي , حيث يقوم بزيارة مجموعة من المواقع و تتبع الروابط التي توجد بها والتي تؤدي بدورها الى مواقع آخرى التي يتم تحليلها أيضا و هكذا, في أثناء قيام محرك البحث بزيارة هذه المواقع يقوم بتحليل الكلمات الموجودة بها و ينشئ قاعدة بيانات تحتوي على نتائج هذا التحليل.

عملية البحث

يطلب محرك البحث من المستخدم ما يسمى بـ “كلمات البحث أو Keywords ـ ” و بناءا على هذه الكلمات يقوم المحرك بالبحث في أرشيفه الخاص و قاعدة البيانات الموجودة لديه و يظهر للمستخدم النتائج.

الشكل العام

تتشابه أغلب محركات البحث في طريقة عرض نتائج البحث مع اختلافات ثانوية في الألوان المستخدمة أو نوع الخط المستخدم , أغلب محركات البحث تعرض النتائج كما هو موضح في الشكلين 1 و 2 .

كيفية انتقاء كلمات البحث

كما سبق و أوضحنا فمحرك البحث لا يطلب من المستخدم أي مدخلات غير كلمات البحث و بالتالي فاختيار كلمات البحث هو العامل الوحيد الذي يتحكم في شكل النتائج لذلك كانت أهمية انتقاء كلمات بحث مناسبة, و فيما يلي بعض الإرشادات توضح كيفية انتقاء كلمات البحث.

استخدام كلمات محددة و الابتعاد عن الكلمات العامة.

مثلا عند البحث عن معلومات تخص الـ muscle contraction لا تستخدم الكلمات Physiology ,Medicine بل استخدم كلمة Muscle Contraction مباشرة.

تجنب استخدام الاختصارات

مثلا ARP بدلا من Absolute refractory period الا اذا كان المراد الاستفسار عن معنى الاختصار “يفضل اضافة Stands +for ـ ” أو كان اختصارا متعارفا عليه عالميا مثل CPR ـ.

ملاحظة عدد نتائج البحث

لأنها تعكس مدى شعبية هذا الموضوع و درجة توثيقه, فاذا كانت قليلة “أقل من مائة ألف مثلا ” في الموضوعات البسيطة مثل الـ Muscle Contraction فلابد من التأكد من صحة كلمات البحث.

كلمات البحث في المثال الأول صحيحة تماما , في الحالة الثانية كل كلمة صحيحة لغويا لكن الجمع بين الكلمتين ليس شائعا ما أدي الى قلة نتائج البحث , أما في الحالة الثالثة فكلمات البحث بها خطأ املائي الأمر الذي أدي الى انخفاض هائل في عدد النتائج

اذاكانت النتيجة غير مرضية “كأن كانت النتائج عامة” ينصح بتضييق نطاق البحث باضافة بعض الكلمات المخصصة للموضوع.

اذا لم تكن نتائج البحث المعروضة في أول ثلاث صفحات مرضية فالأفضل تغيير كلمات البحث.

استخدام معاملات البحث “علامات التنصيص , اشارة الجمع و الطرح”

علامات التنصيص Double quotation ( ” )

في الوضع الطبيعي يحاول محرك البحث ايجاد كلمات البحث في أقرب حالة تقارب, فمثلا عند البحث عن Muscle Contraction سيتم تقديم النتائج التي تحتوي على الكلمتين معا في حالة تقارب,أي ليس بينهما فاصل, هذا الوضع يختلف من محرك بحث الى آخر حسب طريقة عمله.

باستعمال علامات التنصيص يتم اجبار محرك البحث على ايجاد كلمات البحث الموجودة داخل علامات التنصيص كما هي, بنفس الترتيب ودون فواصل بين هذه الكلمات.

أمثلة:

كلمات البحث “وقال ربكم ادعوني أستجب لكم”

ستكون النتائج لمواقع تحتوي على هذا الجزء من الآية الكريمة.

كلمات البحث

“THE TERM Embryology, in its widest sense” وهي جملة من كتاب Gray’s Anatomy

ستكون نتائج البحث لمواقع تعرض هذا الكتاب.

كلمات البحث “All that lives must die” و هي جملة من مسرحية هملت.

ستكون أغلب نتائج البحث لمواقع تعرض أجزاء من المسرحية أو المسرحية بأكملها.

اشارة الطرح ( – )

تستخدم هذه الإشارة لإجبار محرك البحث على عدم عرض النتائج المحتوية على كلمة معينة.

مثال

: APR –Network

سيقوم محرك البحث بالبحث عن كلمة ARP و بعد ذلك سيتخلص من النتائج المحتوية على كلمة Network , كلمة ARP اختصار مشهور في مجال شبكات الحاسوب و بالتالي فنتائج البحث ستكون أغلبها ذات علاقة بعلم الشبكات, ولكن باستعمال معامل الطرح تم استثناء النتائج المحتوية على كلمة Network من النتائج.

اشارة الجمع ( + )

في الوضع الطبيعي قد تتجاهل محركات البحث حروف الجر و الضمائر وأدوات الاستفهام “حتى وان تمت كتاباها في كلمات البحث” لأنها ليست مختصة بموضوع معين فاستعمالها دارج في جميع جوانب الحياة و بالتالي فوجودها يؤثر بالسلب على دقة نتائج البحث.

اشارة الجمع تجبر محرك البحث على عدم تجاهل أحد هذه الكلمات

مثال ARP Stands +for ستقوم باجبار محرك البحث على ادراج for في كلمات البحث

ملاحظة: نظرا للتطوير المستمر في محركات البحث الذي أدى الى ارتفاع في مستوى “ذكاء” هذه المحركات أصبحت عملية تجاهل حروف الجر و غيرها متوقفة على سياق البحث و مدى تأثره بوجود هذه الحروف.

يمكن استعمال أكثر من معامل في نفس الوقت , مثل

“Complete statement” +For +Where -Network

هذا المثال ليس له معنى ولكنه يوضح استعمال أكثر من معامل معا.

بعض الطرق الغير تقليدية في البحث

استخدام محركات البحث في التحقق من صحة عبارة انجليزية معينة

أثناء كتابة موضوع باللغة الإنجليزية , سواء كان خطابا أو مقالة أو بحث , قد تصادف المرئ مشكلة في اختيار حروف الجر الانجليزية التي تتماشى مع فعل معين مثلا Carry out أو Carry forward أوCarry on , أو ربما تواجهك عبارة غريبة مثل

I Take My leave, ربما في الحالة الأولى قد يسعفك القاموس لكن في الحالة الثانية فلا أظن أن القاموس العادي سوف يكون مفيدا.

في هذه الحالة يمكن التأكد من صحة العبارة و التعرف على معناها باستخدام محركات البحث , وذلك بوضعها في علامات تنصيص و ادخالها الى محرك البحث و بالنظر الى عدد النتائج يمكن معرفة مدى صحة العبارة فإن كان عدد النتائج كبيرا فذلك يدل على وجود صفحات كثيرة محتوية على هذه العبارة الأمر الذي يعكس صحتها لغويا , أما للتعرف على معناها فذلك يكون بقراءة بعض نتائج البحث و فهم المعنى من السياق.

في بعض الأحيان قد يكون أحد المواقع في نتائج البحث مختص باللغة و يشرح معنى العبارة مباشرة.

على أية حال, يمكن استخدام محركات البحث كقاموس سواء للمصطلاحات العلمية أو الكلمات الإنجليزية و الأجنبية أو حتى اللغات العامية الأجنبية.

استخدام محركات البحث في اختيار كلمات البحث!

حتى الآن فجميع أمثلة البحث السابقة كانت مشتركة في شيء واحد و هو أن المستخدم يعرف تماما كلمات البحث التي سيتخدمها , و لكن في كثير من الأحوال قد يواجه المستخدم مشكلة كأن رأي تقنية جديدة في أحد برامج التلفاز أو لفت انتباهه طريقة جديدة في ربط ربطة العنق أو غيرها و أراد أن يبحث عنها على الويب, المستخدم يمكنه “وصف” ما رآه و لكنه لا يعرف كلمات البحث التي يجب استخدامها للوصول الى مراده.

في هذه الحالة يمكن استخدام محرك البحث للوصول الى كلمات البحث “و هو اسم التقنية التي رأها في التلفاز على سبيل المثال”.

عملية البحث هنا تكون أشبه باجراء تحري على الويب, لأن المستخدم لا يمتلك الأ بعض الخيوط البسيطة “كوصف التقنية مثلا” ويحتاج الى جمع المزيد من الخيوط ليصل الى هدفه.

فيما يلي مثال يوضح خطوات هذه العملية.

تحذير: هذا المثال لا يتعلق بالعلوم الطبية كما أنه طويل بشكل مبالغ فيه نظرا لتدرة المعلومات المتوفرة عن مادة البحث,الهدف من المثال توضيح أغلب جوانب عملية البحث ولا يعني ذلك أن جميع عمليات البحث لابد أن تمر بهذا العدد الكبير من الخطوات.

في هذا المثال سنحاول البحث عن أحد أفلام الكارتون التي كانت تعرض على القناة الثالثة في أواسط التسعينيات.

الخيوط:

الكارتون ياباني المنشأ (استنتاج من طبيعة الرسوم المستخدمة فيه)

الكارتون كان يدور عن طفل صغير وجد “قمقم” بداخله جني هذا الجني يخرج فقط عندما يعطس مالك “القمقم”.

الكارتون كان قديما بدليل رداءة الصورة.

في أول خطوة بحثت عن Japanese Cartoon

فكان من ضمن النتائج صفحة في الموسوعة الحرة WikiPedia تتحدث عن ما يعرف بالـ Anime.

في هذه الصفحة وجدت التعريف الآتي

The term most popularly refers to animation originating in Japan

أي أن كلمة Anime هي مرادف أكثر شهرة لـ Japanese Cartoon

بعد ذلك بدأت في زيادة تخصيص البحث فبدلا من البحث عن Anime فقط سأبحث عن Anime له علاقة بالعطس Anime Sneeze

النتيجه بوجه عام كانت غير مرضية, ولكن عند اختيار أول نتيجه في نتائج البحث قرأت مقولة غريبة من الثقافة اليابانية مفادها أن الشخص اذا عطس مرة فهذا يعني أن أحدا يذكره بالخير و اذا عطس مرتين فهذا يعني أن أحدا يذكره بالسوء واذا عطس ثلاث مرات فهذا يعني أنه مصاب بنزلة برد!

نواصل البحث , واضح أن كلمات البحث عامة جدا تحتاج الى تخصيص أكثر , قمت باستخدام أحد الخيوط و هي كون الكارتون قديما , فقمت باضافة 60’s 70’s الى كلمات البحث ولكن كانت النتائج غير مرضية “يلاحظ أني لا أتجاوز نتائج الصفحة الأولى”.

النتائج الغير مرضية تعني ضرورة تغيير كلمات البحث , سألجأ لاستخدام خيط آخر و هو كلمة جني باللغة الأنجليزية و تعني Genie وهنا بدأت تظهر نتائج ذات علاقة بالموضوع خصوصا النتيجة الرابعة في صفحة نتائج البحث.

أشارت النتيجة الرابعة الى موقع يتحدث مالكه عن أحد أفلام الكارتون التي كان يفضلها و هو صغير وكانت تدور حول جني شكله مضحك يخرج من القمقم عندما يعطس صاحب القمقم!, تماما ما نبحث عنه و تفضل صاحب الموقع مشكورا بكتابة اسم الكارتون بالإنجليزية ” Hakushon the genie”

و قال أيضا أن كلمة Hakushon هي التعبير الياباني عن صوت العطس, بدأنا نقترب من الهدف النهائي, نقوم باستعمال كلمات Hakushon The Genie فنحصل على نتائج بحث بها هذا الموقع

لاحظت هنا أن كلمة Hakushon جاء بعدها Dai Maoh , لا أفهم معنى الكلمتين ولكن يبدو أنها كلمات ذات علاقة بالهدف “في الأغلب تعني The Genie باليابانية”, قمت بكتابة كلمات البحث الجديدة في محرك البحث وحصلت على نتائج قليلة 162 نتيجه ولكن المحرك نبهني الى احتمال وجود خطأ املائي , و عند تصحيح الخطأ وصل عدد النتائج الى 31,000 ـ

كانت أول النتائج تشير الى موقع يحتوى على كلمات أغنية المقدمة باللغة اليابانية لهذا الكارتون

و عند استخدام بحث الصور Image Search توصلت الى بعض المواقع التي تعرض صور لهذا الكارتون ولكن عدد النتائج كان قليلا جدا 13 نتيجه فلجأت الى تقليص كلمات البحث من Hakushon dia Maoh الى Hakushon فقط.

13 نتيجه فقط

وصل عدد النتائج الى 181 نتيجه, رقم لا بأس به بالنسبة لكارتون قديم من الستينيات, في أثناء تصفح الصور اخترت احدى الصور التي أعجبتني فأوصلني ذلك الى موقع يحتوى على رابط يسمح بتحميل ملف فيديو لمقدمة هذا الكارتون مع أن كل ما أردته هو الوصول الى اسم الكارتون فقط

هناك بعض الأمور التي يجب ملاحظتها:

على عكس المتوقع,هذه العملية لا تستغرق ساعات, فعملية بحث صعبة نسبيا مثل تلك المذكورة في المثال استغرقت حوالى 25-30 دقيقة لا أكثر, وكلما ازدادت شهرة الموضوع و الخيوط التي يمتلكها المستخدم كلما قلت مدة عملية البحث.

سبب النقص الواضح في النتائج الخاصة بهذا الكارتون و بالتالي طول مدة البحث عنه يرجع الى ندرته و قدمه الشديدين , هناك موضوعات قد تكون أكثر شهرة منه و يمكن استعمال نفس طريقة البحث ولكن الوصول الى الهدف سيكون أسرع.

قدرات المستخدم في اللغة الإنجليزية تزيد من سرعة العملية “لقدرته على استخدام كلمات بحث متنوعه” , كذلك فعمليات البحث المركبة مثل المذكورة في المثال تزيد من قدرات المستخدم اللغوية الى حد كبير على المدى الطويل.

على الرغم من اتساع الانترنت يمكن أن تفشل في إيجاد ما تريد حتى باستعمال طرق مركبة ببساطة لأن الموضوع الذي تبحث عنه غير موجود بشكل كاف على الشبكة بالطبع هذا أمر نادر ولكنه ممكن.

الطريقة المتبعة ليست إلا تجربة و خبرة شخصية يكتسبها مستخدمي محركات البحث, يمكن لأي شخص ابتكار أساليبه الخاصه في الوصول الى ما يبحث عنه.

ماذا تعني؟

ويكيبيديا هى موسوعة حرة، متعددة اللغات، يساهم فيها الملايين من المتطوعين حول العالم. تكمن قوة موسوعة ويكيبيديا في نظام الويكي ، حيث يسمح لك هذا النظام بالتعديل وإضافة الصفحات بحرية كاملة.

كلمة ويكيبديا تتكون من جزئين , ويكي Wiki و بيديا Pedia وهي دمج بين كلمتين ,Wiki و Encyclopaedia أي موسوعة مبنية باستعمال تقنية الـويكي

ويكي Wiki: الويكي هو نظام يستخدم في بعض مواقع الإنترنت يسمح لعدد من الأشخاص بالمشاركة في كتابة موضوع أو عدة مواضيع معينة، أغلب مواقع الويكي تسمح لأي شخص بالتعديل في محتواها بحرية.

الموسوعة: هي محاولة لتوثيق و جمع المعرفة الإنسانية. تحتوي الموسوعات على مقالات أو مواضيع في مجالات متعددة (إذا كانت موسوعة عامة) أو مقالات متعلقة بموضوع معين (إذا كانت موسوعة متخصصة، في الطب مثلا).

الحرة: الحرية هنا تعني أنك تستطيع نسخ أي جزء من ويكيبيديا ونشرة في مكان آخر و/أو تعديله بحرية وبدون قيود، كل ما عليك هو أن تذكر أن مصدر هذا النص هو ويكيبيديا. يمكنك أن تطبع ويكيبيديا في كتاب وتبيعه، ولو حصلت على نص من ويكيبيدا مكتوبا فيمنكنك نسخة بأي عدد من النسخ بكل حرية ودون قيود. لو كنت تكتب بحثا فيمكنك الإستعانة بويكيبيديا كمصدر لمعلوماتك. آخرا وليس أخيرا، يمكنك أن تساهم في ويكيبيديا بكل حرية.

تاريخ الويكيبديا

بدأ مشروع ويكيبيديا في 15 يناير 2001، ويوجد اليوم أكثر من 5.5 مليون مقال في الموسوعة في كافة اللغات، منها أكثر من 1.5 مليون مقالة في الموسوعة الإنجليزية وحدها. بدأت النسخة العربية للموسوعة الحرة في يوليو 2003 و تحتوي حاليا على 23 ألف مقالة، ولا تزال الموسوعة العربية في مرحلة البناء.

قوة تقنية الويكي

تكمن قوّة الويكي في حرِّيته؛ فيستطيع أي شخص أن يكتب موضوعا أو يعدِّل موضوعا موجودا دون أي قيود. لو نظرنا للأمر نظرة تقليدية (نظام هرمي، وإدارة، وحدود على كل شيء الخ) فسنعتقد أن الويكي نظام فاشل في الغالب، لكن الويكي يستمد قوته من المجتمع؛ فيتم التركيز على إصلاح الأخطاء بدلا من منعها. نعم، يمكنك أن تكتب مقالة وأن تحتوي على أخطاء؛ فستأتي أنت غدا وتلاحظ هذه الأخطاء وتصلحها أو أراها أنا وأصلحها أو يراها شخص آخر وهكذا. النتيجة أن لدينا محتوى راجعه عشرات وأحيانا مئات (متوسط مراجعة المقالة في ويكيبيديا العربية -وهي ذات نشيط قليل نسبيا- 5.1 مرة لكل صفحة، و 14.72 في النسخة الإنجليزية) الأشخاص فيصبح لدينا محتوى موثوق تم تدقيقه.

مالذي يمنع التخريب اذن؟ لا شيء، كما يستطيع شخص أن يخرب يستطيع آخر أن يعدل، ومادام عدد الأشخاص حسني النية أكبر من عدد الأشخاص سيئي النية فالنتيجة تكون سرعة قياسية في إصلاح الأخطاء.

Constipation treatment

Dietary fiber (bulk-forming laxatives)

The most common sources of fiber include:

  • fruits and vegetables,
  • wheat or oat bran,
  • psyllium seed (for example, Metamucil, Konsyl),
  • synthetic methyl cellulose (for example, Citrucel), and
  • polycarbophil (for example, Equilactin, Konsyl Fiber)

Lubricant laxatives

Lubricant laxatives contain mineral oil as either the plain oil or an emulsion (combination with water) of the oil. The oil stays within the intestine, coats the particles of stool, and presumably prevents the removal of water from the stool. This retention of water in the stool results in softer stool. Mineral oil generally is used only for the short-term treatment of constipation since its long-term use has several potential disadvantages

mollient laxatives (stool softeners)

Emollient laxatives are generally known as stool softeners. They contain a compound called docusate (for example, Colace). Docusate is a wetting agent that improves the ability of water within the colon to penetrate and mix with stool. This increased water in the stool softens the stool. Studies, however, have not shown docusate to be consistently effective in relieving constipation. Nevertheless, stool softeners often are used in the long-term treatment of constipation. It may take a week or more for docusate to be effective. The dose should be increased after one to two weeks if no effect is seen

Hyperosmolar laxatives

Hyperosmolar laxatives are undigestible, unabsorbable compounds that remain within the colon and retain the water that already is in the colon. The result is softening of the stool. The most common hyperosmolar laxatives are lactulose (for example, Kristalose), sorbitol, and polyethylene glycol (for example, MiraLax). and are available by prescription only. These laxatives are safe for long-term use and are associated with few side effects.

Saline laxatives

Saline laxatives contain non-absorbable ions such as magnesium, sulfate, phosphate, and citrate [for example, magnesium citrate (Citroma), magnesium hydroxide, sodium phosphate). These ions remain in the colon and cause water to be drawn into the colon. Again, the effect is softening of the stool

Stimulant laxatives

Stimulant laxatives cause the muscles of the small intestine and colon to propel their contents more rapidly. They also increase the amount of water in the stool, either by reducing the absorption of the water in the colon or by causing active secretion of water in the small intestine

Stimulant laxatives are very effective, but they can cause severe diarrhea with resulting dehydration and loss of electrolytes (especially potassium). They also are more likely than other types of laxatives to cause intestinal cramping. There is concern that chronic use of stimulant laxatives may damage the colon and worsen constipation, as previously discussed. Bisacodyl (for example, Dulcolax, Correctol) is a stimulant laxative that affects the nerves of the colon which, in turn, stimulate the muscles of the colon to propel its contents. Prunes also contain a mild colonic stimulant.

Suppositories

There are stimulant suppositories containing bisacodyl (for example, Dulcolax). Glycerin suppositories are believed to have their effect by irritating the rectum

Combination products

ex.e senna and psyllium (Perdiem), senna and docusate (Senokot-S), and senna and glycerin (Fletcher’s Castoria).

Colchicine is a drug that has been used for decades to treat gout. Most patients who take colchicine note a loosening of their stools. Colchicine has also been demonstrated to relieve constipation effectively in patients without gout.

Misoprostol (Cytotec)

Misoprostol (Cytotec) is a drug used primarily for preventing stomach ulcers caused by nonsteroidal antiinflammatory drugs such as ibuprofen. Diarrhea is one of its consistent side-effects. Several studies have shown that misoprostol is effective in the short term treatment of constipation. Misoprostol is expensive, and it is not clear if it will remain effective and safe with long-term use. Therefore, its role in the treatment of constipation remains to be determined.

Orlistat (Xenical)

Orlistat (Xenical) is a drug that is used primarily for reducing weight. It works by blocking the enzymes within the intestine that digest fat. The undigested fat is not absorbed, which accounts for the weight loss. Undigested fat is digested by bacteria within the intestine and the products of this bacterial digestion promote the secretion of water. The products of digestion also may affect the intestine in other ways, for example, by stimulating the intestinal muscles. In fact, in studies, orlistat has been shown to be effective in treating constipation. Orlistat has few important side effects, which is consistent with the fact that only very small amounts of the drug are absorbed from the intestine.

It is unclear if these prescribed drugs should be used for the treatment of constipation. Although it is difficult to recommend them specifically just for the treatment of constipation, they might be considered for constipated individuals who are overweight, have gout, or need protection from nonsteroidal antiinflammatory drugs.

Your medical history and other medications you’re taking may limit your laxative options. Laxatives can interact with blood thinners such as warfarin (Coumadin), antibiotics such as tetracycline and ciprofloxacin (Cipro), and certain heart and bone medications. Before using any laxative, read the label carefully. If you’re not sure whether a particular laxative is right for you, ask your pharmacist or doctor. Don’t exceed recommended dosages unless your doctor tells you otherwise.

Complicating conditions
Just because laxatives are available without a prescription doesn’t mean that they’re without risk. Laxative use can be dangerous if constipation is caused by a serious condition, such as appendicitis or a bowel obstruction. If you frequently use certain laxatives over a period of weeks or months, they can decrease your colon’s natural ability to contract and actually worsen constipation. In severe cases, overuse of laxatives can damage nerves, muscles and tissues of your large intestine.

Unsafe if you’re pregnant or under age 6
Don’t give children under age 6 laxatives without a doctor’s recommendation. If you’re pregnant, get your doctor’s advice before using laxatives. Bulk-forming laxatives and stool softeners are generally safe to use during pregnancy, but stronger laxatives can harm you or your baby. The stimulant laxative castor oil, for example, can cause womb contractions. If you’ve recently given birth, consult your doctor before using laxatives. Although laxatives are usually safe to use during breast-feeding, some ingredients may pass into breast milk and cause diarrhea in nursing infants.

Don’t be lax about laxatives

Talk to your doctor before using laxatives if you have the following:

  • Kidney disease
  • Heart disease or high blood pressure
  • Stomach pain, nausea, vomiting or rectal bleeding

Call your doctor if you’re taking laxatives and have these symptoms:

  • Bloody stools
  • Constipation doesn’t improve after a week of laxative use

If you’re dependent on laxatives, ask your doctor for advice on how to gradually withdraw from them and restore your colon’s natural ability to contract.

Icariin from horny goat weed is structurally unrelated to sildenafil (Viagra®)


In our last post we discussed a recent USPTO ruling that rejected a claim of the Pfizer patent on the erectile dysfunction drug, sildenafil (Viagra®), to a novel oral treatment for the disorder. The patent appeals panel ruled that the existence of horny goat weed, a traditional Chinese medicine used orally for impotence, was grounds for rejection of the claim.

Frequent commenter daedalus4u pulled out US Patent #6,469,012 (filed Mar 4 1996, issued Oct 22 2002) wherein the relevant claim 24 reads:

A method of treating erectile dysfunction in a male human, comprising orally administering to a male human in need of such treatment an effective amount of a selective cGMP PDE inhibitor, or a pharmaceutically acceptable salt thereof, of a pharmaceutical composition containing either entity.

The claim should never have been allowed in the first place, said daedalus4u, and if upheld would have allowed Pfizer to block sales of horny goat weed. I'm also assuming that upholding claim 24 would have also allowed Pfizer to block sales of follow-on PDE5 inhibitors such as Lilly's Cialis®. But again, I'm not a patent attorney so I welcome further discussion on this point.

We also heard from my dear old colleague from Adelaide, Professor Ian Musgave. Ian is a pharmacologist who is probably better known in the blogosphere for his astronomy expertise (Astroblog, Southern Sky Watch, and contributing editor to Sky & Space).

But Ian is first and foremost a pharmacologist and raised this point:

Wait a minute, let me get this straight. The patent on Sildenafil, a compound that was rationally designed to specifically inhibit PDE (1) and developed prior to 1996 was invalidated because of a structurally unrelated component of a herbal extract that wasn't even shown to act on the cGMP/NO system before 2001? (2)

Pfizer is supposed to have time travel? The fact that a herbal treatment alleged to help sexual function would turn out, 5 years after the sildenafil patent had been granted (and the concept of PDE5 inhibitors as treatment for erectile dysfunction wide spread) to have effects on PDE5 could not possibly count as prior art. If so, the 1983 demonstration that the PDE inhibitor papaverine could produce erections would have invalidated the sildenafil patent.

The patent boards ruling is patently rubbish. How can sildenafil "logically flow" from a compound that wasn't even known to really work, let alone known to inhibit PDE?

Prof Musgrave is indeed correct in the argument but daedalus4u points out the ruling did not invalidate the Pfizer patent, just the single claim to the pharmaceutical indication.

So that makes more sense to me and now I think I understand.

But in the discussion, Ian gave us another gem in response to a commenter who argued there was a structural resemblance between sildenafil and icariin, the active flavonol glycoside constituent of horny goat weed:

No, not at all. You can stick the benzofuran of the flavonol on top of the methylxanthine moiety of the sildenafil, but the charge distribution and 3D structure is all wrong, the flavonol has these huge sugar moieties which have the wrong charge and stick out in places that should inhibit binding, it lacks the bulky positively charged ring that fits into the PDE5 specific binding pocket. (I have the an image of the 3D structural overlay if anyone want to see them).

Looking at it, icariin does not scream either methylxanthine mimetic or "PDE inhibitor", in fact I would guess it would be worthless. However, flavonols and flavinoids have a habit of doing things we don't expect, such as resveratrol being a very decent cannaboid receptor anatgonist, despite looking nothing like cannabinoids of the synthetic antagonists.

In fact, Ian was kind enough to send us two molecular modeling images of the two compounds which I failed to get posted last night that illustrate the vast difference between sildenafil and icariin. This overlay image was generated with RASMOL, with icariin in the wireframe format and sildenafil in stick format:

We reported yesterday that icariin has only 1/80th the potency of sildenafil in inhibiting PDE5 and its maximal effect on cGMP accumulation in an rabbit tissue model is 1/10th that of sildenafil. One doesn't need to be a computational chemist to see from Ian's overlay why this difference in potency and efficacy exists.

Anyway, this has been an interesting scientific exchange on an issue that was largely only covered in the pharmaceutical business press. This also illustrates for me the fun of science blogging - we all come to the table, albeit the electronic table, with our own expertise, have a discussion and learn from one another, even if some of us are halfway around the world.

Horny Goat Weed (Epimedium spp.) is a limp excuse for Viagra, Cialis


Reuters and Bloomberg reported earlier this week on an ongoing patent battle (read: pissing match) between Pfizer and Eli Lilly & Co. relating to their erectile dysfunction drugs Viagra and Cialis, respectively.

Goat.jpgA US Patent and Trademark Office (USPTO) appeals committee has ruled that an element of Pfizer's patent on sildenafil, the active chemical in Viagra, is invalid because the drug is insufficiently different from a traditional Chinese medicine called Yin Yang Huo or horny goat weed.

At issue is Pfizer's claim to a method for treating male erectile dysfunction. The patent appeals panel ruled that the method did not constitute a new invention because of the precedent set by Yin Yang Huo. Moreover, the board ruled that chemicals found in the herbal medicine act by the same mechanism as sildenafil by inhibition of an enzyme called phosphodiesterase-5 (PDE5). Therefore, the panel ruled, "the patent claim was the next logical step up from using the herb."

I'm not an expert in law but there are untold number of traditional remedies touted for all sorts of sexual enhancement, none of which have the convincing efficacy of the prescription PDE5 inhibitors. We may call the condition "erectile dysfunction" today and the idea of treating it may have existed for centuries but having a compound that can actually do anything about it is an invention. However, I can see the fine distinction if Pfizer claimed that the idea of treating erectile dysfunction was an invention.

You lawyers out there can weigh in but this sounds like a bunch of posturing for market share: worldwide Viagra sales were $2 billion USD last year.

But what is this horny goat weed and why is it being singled out?

Horny goat weed is the colloquial name for any number of species of plants in the Epimedium genus. Most medicinal plant accounts describe some variation on the theme that a Chinese goat herder observed an increase in mating behavior among his flock while grazing on the plant. According to North Carolina State University Department of Horticulture webpage, it grows as a deciduous perennial ground cover in USDA Zones 6, 7, and 8.

Moreover, icariin is a poor inhibitor of PDE5, with a IC50 value of 5.9 μM, making it about 80-fold less potent than sildenafil (IC50 = 75 nM) (J Nat Prod 2008; 71:1513-1517.)

And remember: that's the purified compound in an isolated enzyme assay. Let's take a look at the plant extract itself, the product sold on the internet and in health food stores as horny goat weed.

Horny Goat Weed Fig 4 Int J Impot Res.jpgThis figure comes from a 2006 paper in the International Journal of Impotence Research (2006; 18:335-342) where an extract of a specifically-selected Epimedium plant was extracted and partially purified, then tested relative to sildenafil in a tissue preparation of rabbit corpus cavernosum (the crucial penile vasculature). The extract, EP-20 was tested here for its ability to increase the levels of the second-messenger molecule cGMP in response to the nitric oxide donor, sodium nitroprusside (SNP). Erectile dysfunction agents acts to inhibit the degradation of cGMP, causing the corpus cavernosum to relax and increase blood flow to the penis.

The EP-20 extract does indeed increase SNP-stimulated cGMP concentration over SNP alone, but it tops out at about 1/10 the levels stimulated by sildenafil (note the broken y-axis for scale). More importantly, the concentrations of horny goat weed extract are tremendously high: 0.1 and 0.3 mg/mL. A direct comparison with sildenafil is difficult because it is a pure compound while the EP-20 horny goat weed extract is a mixture of all the chemicals that occur in the plant.

The take-home messages?:


  • The active constituent of horny goat weed, icariin, bears no structural similarity to sildenafil.

  • Sildenafil is 80 times more potent than icariin in inhibiting phosphodiesterase-5

  • Horny goat weed extract must be used at unbelievably high concentrations - far more than can be achieved in the bloodstream - to cause only 1/10th the effectiveness of sildenafil in a rabbit penile blood vessel model.

So, horny goat weed is far from stiff competition for a prescription PDE5 inhibitor like sildenafil. Yes, it has the potential to target an animal model of penile vasculature in a manner similar to sildenafil but it is unlikely to occur in a real-life scenario. However, there have been no head-to-head clinical trials of horny goat weed extract and sildenafil.

Going back to the Pfizer and Eli Lilly spat, there is really little comparison between horny goat weed and the active compound in Viagra.

And as for anyone thinking that horny goat weed might be a more cost-effective substitution for Viagra? You're probably better off buying a dietary supplement intentionally adulterated with other prescription drugs.
